Consent Forms

Informed consent is an essential part of safe and effective treatment. Before each treatment, particularly those involving needles, lasers, peels or active ingredients, your therapist will ask you to read and sign a treatment-specific consent form. This page explains what to expect and why we ask.

1. Why consent matters

A signed consent form confirms that:

  • You understand what the treatment involves,
  • You have been informed of the expected outcomes, the possible side effects, and the contraindications,
  • You have disclosed all relevant medical history, medications and allergies,
  • You agree to follow the aftercare instructions provided.

Consent is required by Australian health practice standards and protects both you and our therapists.

2. Treatments that require written consent

We use written consent for all treatments that involve any of the following:

  • Needles or skin perforation (microneedling, salmon DNA microneedling, cosmetic tattoo, lip blush, lip liner, wrinkle reducing treatments).
  • Energy-based devices (HydraFacial, laser hair removal, laser skin tightening, LED light therapy, radio-frequency microneedling).
  • Chemical or enzyme peels of any depth (Aura Glass Skin Peel, IS Clinical Prodigy Peel Elite, Shinning Peel, Snow Peel).
  • Cosmeceutical product application at clinical strength.

Eyebrow tinting, eyelash tinting, threading, brow lamination and gentle facials do not require written consent but still include a verbal pre-treatment discussion and patch test where appropriate.

3. Patch testing

We patch test for tinting and certain peel products at least 24–48 hours before first use. A patch test does not eliminate all risk, but it significantly reduces the chance of an unexpected reaction.

4. Medical history and medications

You must disclose:

  • Pregnancy or breastfeeding status,
  • Current medications (especially Roaccutane, antibiotics, blood thinners, immunosuppressants),
  • Recent cosmetic procedures (injectables, peels, microneedling, laser within the past 6 weeks),
  • Allergies and sensitivities,
  • Skin conditions (eczema, psoriasis, rosacea, active cold sores, recent sunburn).

Failure to disclose relevant history may make the treatment unsafe or void our ability to address adverse outcomes.

5. Right to withdraw consent

You may stop a treatment at any time. Simply tell your therapist. We will pause and discuss next steps, including a refund of any unused treatment time on a pro-rata basis.

6. Consent for under-18s

For clients under 18 years of age, a parent or legal guardian must:

  • Attend the appointment,
  • Sign the consent form on the client's behalf,
  • Confirm they are aware of the treatment and any risks.

Some treatments are not offered to clients under 16. Please call us beforehand to confirm.

7. Photography and before/after images

A separate consent is asked for clinical photography. You may attend treatments without consenting to photography. Where you do consent, you specify whether images may be used:

  • For your own clinical record only,
  • For Aura Skin Boutique's portfolio, with no identifying features,
  • For marketing on our website and social media, with or without identifying features.

You may withdraw photography consent at any time, in writing, and we will remove published images within 14 days.

8. Storage of consent forms

Signed consent forms are stored securely at our Essendon premises (paper) and within our protected Timely client record (digital). See our Privacy Policy for full details.
